CONFERENCE PROGRAM DESCRIPTION
The nine-day conference agenda is yet to be finalized, although it is intended to focus on current health related issues, trends practiced and professional challenges facing today’s physicians.
The Clinical Medical Education categories to be addressed in this year’s conference will include:
The practice of Family Medicine in a rapidly changing world along with specialty medicine subjects addressing Pediatric Medicine; Orthopedic Medicine; Gastroenterological Problems; General Surgery; Dermatological Disorders; Obstetrics and Gynecology; Infertility; Urology Medicine; Psychiatric Disorders; and General Anesthesia.
CONFERENCE COST AND TUITION
This year’s airfare and hotel package cost is $2,521.00, per person based on double occupancy. Single, triple and children’s options are available.

- Roundtrip airfare from Des Moines to Kauai on United Airlines (Group dates, January 22 – 31, 2009).
- Nine nights/ten days at Grand Hyatt Kauai Resort & Spa – Garden view rooms
The Garden View Rooms offer lush landscaped garden views from a private patio. Each room offers one king or two queen Grand Beds™. These spacious accommodations include inviting amenities such as iHome stereo with iPod dock, beverage chillers, coffeemaker, Hawaiian Kona coffee, large work area and wireless Internet access. Upgraded room types are available upon you request and at a higher rate.

Registration Fee:
- Please send the conference registration fee with your initial deposit.
- Physician Fee: $280.00
- Registered Nurse Fee: $100.00

CONFERENCE REGISTRATION AND DEPOSIT SCHEDULE
- If you plan to attend the conference presentations, please complete the registration form and a $280.00 conference fee for physician, $100.00 fee for nurses and $100.00 fee for any other meeting attendees.
- A $400.00 nonrefundable deposit should be mailed on or before September 24, 2008. Receipt of your deposit will secure your reservation for the conference.
- Deposit due date—September 24, 2008
- Final payment due date—November 24, 2008
CANCELLATION POLICY
- September 24—October 10, 2008—$400.00 per person deposit is nonrefundable
- October 11—December 10, 2008—$800.00 per person is nonrefundable
- December 11, 2008—Departure—Full amount nonrefundable +
*Note* Once your airline tickets are issued they are non-refundable. If you cancel your trip after your airline tickets have been issued, you can use these tickets towards the purchase of a new ticket on the same airline with-in one year from the date of purchase. Additional fees/cancellation fees will apply.
