We encourage you to attend the 2008 Iowa Lutheran Hospital Medical Staff Mid-Winter Seminar and discover the natural beauty and splendor of Hawaii’s largest island for yourself.
CONFERENCE PROGRAM DESCRIPTION
The nine-day conference agenda is yet to be finalized, although it is intended to focus on current health related issues, trends practiced and professional challenges facing today’s physicians.
The Clinical Medical Education categories to be addressed in this year’s conference will include: The practice of Family Medicine in a rapidly changing world along with specialty medicine subjects pertaining to Pediatric Medicine; Orthopedic Medicine; General Surgery; Obstetrics and Gynecology; Infertility; Urology Medicine; Psychiatric Disorders; and General Anesthesia.
CONFERENCE COST AND TUITION
This year’s airfare and hotel package cost is $2,410.00, per person based on double occupancy. Single, triple and children’s options are available. Please contact Jenny Serena (515-273-3317 or 800-550-1977ext. 317) with TravelFlo for your travel needs.
Package price includes:
- Roundtrip airfare from Des Moines to Kona, Hawaii on United Airlines (group dates only, January 24 – February 2, 2008).
- Nine nights/ten days at The Fairmont Orchid Resort Room Category, The Fairmont Guest Rooms offer a spacious accommodation of a King or 2 Queen size beds, private lanai with a view of the hotel gardens. Upgraded room types are available upon you request and at a higher rate.
- Hotel Tax
- Roundtrip hotel porterage fees
- TravelFlo Staff to assist you with activities while on the Big Island of Hawaii.
Registration Fee:
- Please send the conference registration fee with your initial deposit.
- Physician Fee: $255.00
- Registered Nurse Fee: $100.00
- Other conference attendees: $100.00

CONFERENCE REGISTRATION AND DEPOSIT SCHEDULE
- If you plan to attend the conference presentations, please complete the registration form and a $255.00 conference fee for physician, $100.00 fee for nurses and $100.00 fee for any other meeting attendees.
- A $400.00 nonrefundable deposit should be mailed on or before September 24, 2007. Receipt of your deposit will secure your reservation for the conference.
- Deposit due date—September 24, 2007
- Final payment due date—November 24, 2007
CANCELLATION POLICY
- September 24—October 10, 2007—$400.00 per person deposit is nonrefundable
- October 11—December 10, 2007—$800.00 per person is nonrefundable
- December 11, 2007—Departure—Full amount nonrefundable +
*Note* Once your airline tickets are issued they are non-refundable. If you cancel your trip after your airline tickets have been issued, you can use these tickets towards the purchase of a new ticket on the same airline with-in one year from the date of purchase. Additional fees/cancellation fees will apply.
